Summary

Posaconazole may increase propafenone concentrations by inhibiting CYP3A4, and the risk is greater in CYP2D6 poor metabolisers or those taking CYP2D6 inhibitors. Higher levels raise the risk of proarrhythmia, conduction disturbances and CNS effects, and both drugs may affect the QT interval.

What this interaction means in plain words

Posaconazole and Propafenone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Posaconazole belongs to Anti Fungals; Propafenone belongs to Cardiovascular.

Evidence level: Low

Why it happens

Inhibition of CYP3A4 reduces propafenone N-dealkylation, with possible additive QT/conduction effects.

What to do

Avoid if possible; if used together, monitor ECG (QRS and QT) and consider reducing the propafenone dose.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
