Summary
Posaconazole can raise quinidine concentrations and both agents prolong the QT interval. The result is a substantial risk of torsades de pointes and other serious ventricular arrhythmias. Labeling contraindicates the combination.
What this interaction means in plain words
Posaconazole and Quinidine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Posaconazole belongs to Anti Fungals; Quinidine belongs to Anti Parasitic.
Why it happens
Posaconazole inhibits CYP3A4 metabolism of quinidine, and both drugs have additive QT-prolonging effects.
What to do
Avoid the combination and select an alternative antifungal or antiarrhythmic.
