Summary

Ritonavir strongly inhibits CYP3A4 and also inhibits CYP2D6, and may raise prochlorperazine concentrations. Higher levels could increase sedation, extrapyramidal effects and QT prolongation, and ritonavir itself can prolong QT.

What this interaction means in plain words

Prochlorperazine and Ritonavir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Prochlorperazine belongs to Central Nervous System; Ritonavir belongs to Anti Virals.

Evidence level: Low

Why it happens

Inhibition of CYP2D6 and CYP3A4 metabolism of prochlorperazine, with possible additive QT effects.

What to do

Use the lowest effective prochlorperazine dose and monitor for sedation, extrapyramidal symptoms and palpitations. Consider an ECG in patients with other QT risk factors.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
