Summary
Torsemide-induced hypokalemia and hypomagnesemia increase the risk of QT prolongation and torsades de pointes with sotalol. This is clinically important because sotalol has dose-dependent proarrhythmic effects.
What this interaction means in plain words
Sotalol and Torsemide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Sotalol belongs to Cardiovascular; Torsemide belongs to Cardiovascular.
Why it happens
Loop diuretic electrolyte depletion augments the QT-prolonging action of sotalol, a class III antiarrhythmic.
What to do
Correct potassium and magnesium before starting sotalol and monitor electrolytes and QTc regularly. Keep potassium in the normal range and reassess if QTc lengthens.
What to ask your pharmacist
