Summary
Both agents suppress immune function, so concurrent use increases the risk of serious infection and possibly malignancy. Tacrolimus is a CYP3A4 substrate and combined mTOR inhibitor and calcineurin inhibitor use can also enhance renal toxicity.
What this interaction means in plain words
Tacrolimus and Temsirolimus can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Tacrolimus belongs to Miscellaneous; Temsirolimus belongs to Oncology.
Why it happens
Additive immunosuppression through calcineurin and mTOR pathway inhibition.
What to do
Avoid unless clearly necessary; if combined, monitor for infection, renal function and tacrolimus trough levels.
