Summary

Tipranavir, given with ritonavir, may raise tramadol exposure through CYP3A4 inhibition. Ritonavir also inhibits CYP2D6, which reduces formation of the active metabolite, so analgesia may be less reliable while parent-drug related adverse effects such as seizures and serotonin toxicity may increase.

What this interaction means in plain words

Tipranavir and Tramadol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Tipranavir belongs to Anti Virals; Tramadol belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4 and CYP2D6 by ritonavir-boosted tipranavir alters tramadol metabolism.

What to do

Monitor for reduced analgesia, sedation, seizures and serotonin syndrome. Consider a non-tramadol analgesic if pain control is inadequate.

What to ask your pharmacist
