Summary

Trazodone is metabolized to its active metabolite mainly by CYP3A4, and voriconazole strongly inhibits this enzyme. Higher trazodone levels can cause excess sedation, orthostatic hypotension and QT prolongation, and voriconazole itself can prolong the QT interval.

What this interaction means in plain words

Trazodone and Voriconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Trazodone belongs to Central Nervous System; Voriconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Voriconazole inhibits CYP3A4 metabolism of trazodone, with additive QT effects.

What to do

Start with a lower trazodone dose and titrate by response, watching for sedation and hypotension. Check an ECG if there are other QT risk factors.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
