Résumé

Apraclonidine is an alpha-2 agonist eye drop with limited systemic exposure, but labeling contraindicates its use with MAOIs. MAOIs may alter catecholamine handling and unpredictably change blood pressure responses.

Ce que signifie cette interaction, en termes simples

Apraclonidine and Tranylcypromine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Apraclonidine belongs to Cardiovascular; Tranylcypromine belongs to Central Nervous System.

Niveau de preuve: Low

Pourquoi elle survient

The mechanism is not well established; MAOI-related changes in catecholamine metabolism may alter responses to alpha-agonists.

Conduite à tenir

Avoid apraclonidine in patients on tranylcypromine and select another ocular hypotensive agent such as a prostaglandin analogue or beta-blocker as appropriate. If it is unavoidable, monitor blood pressure and heart rate.
