Summary

Toremifene can cause hypercalcemia, and thiazides reduce urinary calcium excretion, which may add to this risk. Toremifene also prolongs the QT interval, so electrolyte disturbances from the thiazide, such as hypokalemia and hypomagnesemia, are important.

What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Toremifene can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Toremifene belongs to Endocrine.

Evidence level: Moderate

Why it happens

Thiazide-reduced renal calcium excretion combined with toremifene-related hypercalcemia, and thiazide-induced electrolyte loss that increases QT risk.

What to do

Monitor serum calcium, potassium and magnesium, and correct abnormalities. Consider an ECG, and use an alternative antihypertensive if electrolytes cannot be maintained.

What to ask your pharmacist
