Summary
Loop diuretics such as ethacrynic acid cause volume depletion, and adding an ACE inhibitor like perindopril can produce sudden hypotension and a decline in renal function. The risk is highest at initiation, in volume-depleted patients, and in those with renal impairment.
What this interaction means in plain words
Amlodipine-perindopril and Ethacrynic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-perindopril belongs to Cardiovascular; Ethacrynic acid belongs to Cardiovascular.
Why it happens
Additive hypotensive effect, as diuretic-induced volume and sodium depletion increases dependence on angiotensin II and ACE inhibition removes it.
What to do
Monitor blood pressure, renal function and electrolytes after starting or increasing either drug. Consider temporarily reducing the diuretic dose before starting perindopril.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
