Summary

Apalutamide is a strong inducer of CYP3A4 and other enzymes, and can substantially lower vortioxetine concentrations. This may lead to loss of antidepressant effect and relapse.

What this interaction means in plain words

Apalutamide and Vortioxetine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Apalutamide belongs to Endocrine; Vortioxetine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Induction of CYP3A4 (and other CYP enzymes) increases the hepatic clearance of vortioxetine.

What to do

Monitor for reduced antidepressant response and consider increasing vortioxetine, up to about three times the original dose per labeling for strong inducers, but not above the maximum recommended dose. Reduce the dose back within 14 days if the inducer is stopped.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
