Overview

Severity: Moderate · Evidence level: High

Additive increase in calcium absorption and load, raising the risk of hypercalcemia and hypercalciuria when calcium salts are combined with vitamin D.

Mechanism of action

Pharmacodynamic: Vitamin D enhances intestinal calcium absorption; concurrent calcium supplementation increases total calcium availability.

Clinical management

Avoid excessive doses; use the minimum necessary to meet requirements. Monitor serum calcium (and urinary calcium in patients with nephrolithiasis risk) periodically, especially in CKD or when high-dose vitamin D is used. Reduce or hold one or both agents if hypercalcemia develops.
