Summary

Dasatinib is metabolized mainly by CYP3A4, and posaconazole can substantially raise its concentrations, increasing myelosuppression, fluid retention and bleeding risk. Both drugs also prolong the QT interval, adding to arrhythmia risk.

What this interaction means in plain words

Dasatinib and Posaconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Dasatinib belongs to Oncology; Posaconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Posaconazole inhibits CYP3A4 metabolism of dasatinib, and both drugs have additive QT-prolonging effects.

What to do

Prefer an alternative antifungal without strong CYP3A4 inhibition; if combined, reduce the dasatinib dose per labeling and monitor ECG, electrolytes and blood counts.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
