Summary

Disopyramide is a class Ia antiarrhythmic that prolongs the QT interval and can cause torsades de pointes. Desflurane adds QT prolongation, and disopyramide's negative inotropic effect may worsen anesthetic-related hypotension.

What this interaction means in plain words

Desflurane and Disopyramide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Desflurane belongs to Central Nervous System; Disopyramide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive QT prolongation through delayed cardiac repolarization, with additive negative inotropic and hypotensive effects.

What to do

Obtain an ECG and correct potassium and magnesium before anesthesia. Monitor the ECG and blood pressure continuously and have defibrillation available.
