Summary
Dofetilide strongly prolongs the QT interval and carries a substantial risk of torsades de pointes. Beta-2 agonists such as vilanterol can lower potassium and increase heart rate, which may further increase this risk.
What this interaction means in plain words
Dofetilide and Umeclidinium-vilanterol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dofetilide belongs to Cardiovascular; Umeclidinium-vilanterol belongs to Respiratory.
Why it happens
Additive effects on cardiac repolarization together with beta-agonist-induced hypokalemia and sympathetic stimulation.
What to do
Monitor QTc and keep potassium and magnesium within normal range, especially during dofetilide initiation in a monitored setting. Consider an alternative bronchodilator if QT prolongation develops.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
