Summary

Both drugs prolong ventricular repolarization, and giving them together adds to the risk of marked QTc prolongation and torsades de pointes. Dofetilide already carries a substantial proarrhythmic risk, and ziprasidone is labeled against use with other QT-prolonging agents.

What this interaction means in plain words

Dofetilide and Ziprasidone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Dofetilide belongs to Cardiovascular; Ziprasidone belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive blockade of the hERG potassium current (IKr), causing additive QT prolongation.

What to do

Do not combine; choose an antipsychotic with less QT effect if dofetilide is required. If the patient is already on dofetilide, review the need for any QT-prolonging psychotropic.
