Summary
Levoketoconazole causes dose-related QT prolongation and its labeling contraindicates use with other QT-prolonging drugs that are CYP3A4 substrates or otherwise raise torsades risk. Dolasetron adds to this risk of serious ventricular arrhythmia.
What this interaction means in plain words
Dolasetron and Levoketoconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dolasetron belongs to Gastrointestinal Tract; Levoketoconazole belongs to Endocrine.
Why it happens
Additive QT prolongation; levoketoconazole may also inhibit CYP3A4-mediated clearance of dolasetron metabolites.
What to do
Avoid coadministration and use a non-QT-prolonging antiemetic such as one from another class. If unavoidable, correct electrolytes and monitor ECG.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
