Summary

Posaconazole increases eliglustat concentrations, which may prolong PR, QRS and QT intervals and cause arrhythmias. The effect is greater in CYP2D6 poor or intermediate metabolizers and when a CYP2D6 inhibitor is also taken.

What this interaction means in plain words

Eliglustat and Posaconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Eliglustat belongs to Miscellaneous; Posaconazole belongs to Anti Fungals.

Evidence level: High

Why it happens

Posaconazole inhibits CYP3A4, a secondary eliglustat clearance pathway that becomes critical when CYP2D6 activity is low or inhibited.

What to do

Avoid in CYP2D6 poor or intermediate metabolizers and with any additional CYP2D6 inhibitor. In extensive metabolizers, follow labeling with a reduced eliglustat dose and ECG monitoring, or choose an alternative antifungal.

What to ask your pharmacist

  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
