Summary

Elranatamab causes cytokine release syndrome, and inflammatory cytokines can temporarily suppress CYP enzymes. Riluzole is mainly metabolised by CYP1A2, so its exposure and adverse effects such as liver enzyme elevation or nausea may rise during and after CRS.

What this interaction means in plain words

Elranatamab and Riluzole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Elranatamab belongs to Oncology; Riluzole belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Cytokine-mediated suppression of CYP1A2 activity during cytokine release syndrome, raising riluzole exposure.

What to do

Monitor for riluzole adverse effects and liver enzymes during step-up dosing and for about 14 days after the first treatment dose, and during and after CRS. Consider dose adjustment or temporary interruption if toxicity occurs.

What to ask your pharmacist
