Summary
Strong CYP3A4 inhibitors markedly raise eplerenone exposure, increasing the risk of hyperkalemia and associated arrhythmias. Eplerenone labeling contraindicates its use with strong CYP3A4 inhibitors.
What this interaction means in plain words
Eplerenone and Posaconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Eplerenone belongs to Cardiovascular; Posaconazole belongs to Anti Fungals.
Why it happens
Inhibition of CYP3A4 greatly reduces the metabolic clearance of eplerenone.
What to do
Do not combine; use an alternative antifungal or an alternative mineralocorticoid antagonist, and check potassium if exposure has occurred.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
