Summary
Idelalisib is a strong CYP3A inhibitor and is expected to substantially increase ergotamine exposure, risking severe vasospasm and ischemia. Strong CYP3A inhibitors are contraindicated with ergotamine.
What this interaction means in plain words
Ergotamine and Idelalisib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Ergotamine belongs to Central Nervous System; Idelalisib belongs to Oncology.
Why it happens
Idelalisib strongly inhibits CYP3A4, markedly reducing ergotamine clearance.
What to do
Do not co-administer; use a non-ergot migraine therapy during idelalisib treatment.
