Summary

Intravenous erythromycin may raise pravastatin concentrations and increase the risk of myopathy. Pravastatin is not much dependent on CYP3A4, so the effect is smaller than with other statins, but it is still recognised.

What this interaction means in plain words

Erythromycin lactobionate and Pravastatin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Erythromycin lactobionate belongs to Anti Bacterial; Pravastatin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Erythromycin inhibits OATP1B1-mediated hepatic uptake of pravastatin and may increase its intestinal absorption.

What to do

Monitor for muscle pain, weakness or dark urine during the antibiotic course and check CK if symptoms develop. Consider temporarily holding pravastatin during short IV therapy.
