Summary
Palbociclib is a weak time-dependent inhibitor of CYP3A4 and may modestly raise exposure to everolimus, a sensitive CYP3A4 and P-gp substrate. Higher everolimus levels could increase stomatitis, myelosuppression, infections and pneumonitis.
What this interaction means in plain words
Everolimus and Palbociclib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Everolimus belongs to Oncology; Palbociclib belongs to Oncology.
Why it happens
Palbociclib inhibits intestinal and hepatic CYP3A4, reducing the metabolic clearance of everolimus.
What to do
Monitor everolimus trough concentrations where available and watch for mucositis, cytopenias and pneumonitis. Reduce the everolimus dose or interrupt treatment if toxicity develops.
