Summary

Lopinavir (given with ritonavir) is metabolized by CYP3A4, and fluconazole may modestly raise its concentrations. Because ritonavir already strongly inhibits CYP3A4, the added effect of fluconazole is typically small. Both agents can also prolong QT to some degree.

What this interaction means in plain words

Fluconazole and Lopinavir have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Fluconazole belongs to Anti Fungals; Lopinavir belongs to Anti Virals.

Evidence level: Low

Why it happens

Fluconazole inhibits CYP3A4, slightly decreasing lopinavir clearance.

What to do

No dose adjustment is usually required; monitor for lopinavir adverse effects such as GI intolerance and consider an ECG in patients with QT risk factors.
