Summary
Both drugs can prolong the QT interval, so combined use may raise the risk of torsades de pointes, especially with other risk factors. Fluconazole may also modestly raise risperidone exposure through CYP2D6 and CYP3A4 inhibition.
What this interaction means in plain words
Fluconazole and Risperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fluconazole belongs to Anti Fungals; Risperidone belongs to Central Nervous System.
Why it happens
Additive QT prolongation, plus fluconazole inhibition of CYP3A4/CYP2D6 metabolism of risperidone.
What to do
Obtain a baseline ECG and correct potassium and magnesium before combining. Monitor QTc and for increased antipsychotic effects, and reduce the risperidone dose if needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
