Summary

Posaconazole is a strong CYP3A4 inhibitor and also inhibits P-glycoprotein, and futibatinib is cleared in part by both pathways. Coadministration can raise futibatinib exposure and increase the frequency and severity of its toxicities, such as hyperphosphatemia and ocular effects.

What this interaction means in plain words

Futibatinib and Posaconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Futibatinib belongs to Oncology; Posaconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4 metabolism and P-glycoprotein efflux of futibatinib by posaconazole increases futibatinib exposure.

What to do

Avoid the combination where possible and consider an antifungal that does not strongly inhibit CYP3A4 and P-gp. If it cannot be avoided, monitor closely for futibatinib adverse effects and reduce the dose or interrupt treatment as required.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
