Summary

Adding linagliptin to a sulfonylurea such as glipizide raises the likelihood of hypoglycemia compared with the sulfonylurea alone. Linagliptin has little hypoglycemic risk by itself, but it adds to the sulfonylurea's effect.

What this interaction means in plain words

Glipizide and Linagliptin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Glipizide belongs to Endocrine; Linagliptin belongs to Endocrine.

Evidence level: High

Why it happens

DPP-4 inhibition increases incretin levels, enhancing insulin secretion on top of sulfonylurea-stimulated release.

What to do

Consider reducing the glipizide dose when linagliptin is started. Monitor blood glucose and counsel on hypoglycemia.

What to ask your pharmacist
