Summary
Both drugs can prolong the QT interval, so concurrent use may increase the risk of torsades de pointes. Risk is greater in patients with electrolyte imbalance, heart disease, advanced age or other QT-prolonging drugs.
What this interaction means in plain words
Hydroxyzine and Posaconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Hydroxyzine belongs to Respiratory; Posaconazole belongs to Anti Fungals.
Why it happens
Additive QT prolongation; posaconazole may also modestly raise hydroxyzine exposure via CYP3A4 inhibition.
What to do
Avoid the combination where possible and choose a non-QT-prolonging antihistamine. If unavoidable, use the lowest hydroxyzine dose, correct electrolytes and monitor the ECG.
What to ask your pharmacist
