Summary
Levoketoconazole prolongs the QT interval in a dose-related fashion and its labeling contraindicates use with other QT-prolonging drugs that are CYP3A4 substrates or that cause torsades. Combined with sotalol, the risk of torsades de pointes is substantially increased.
What this interaction means in plain words
Levoketoconazole and Sotalol are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Levoketoconazole belongs to Endocrine; Sotalol belongs to Cardiovascular.
Why it happens
Additive QT prolongation through IKr channel blockade.
What to do
Avoid the combination; consider alternative cortisol-lowering therapy or an alternative antiarrhythmic or beta-blocker. If it cannot be avoided, specialist supervision with ECG and electrolyte monitoring is essential.
