Summary
Sodium polystyrene sulfonate exchanges sodium for potassium in the gut and can also bind divalent cations such as magnesium, which lowers serum magnesium rather than raising it. Hypomagnesemia can occur, particularly with prolonged use and when magnesium intake is reduced, and may add to the electrolyte disturbances the resin already causes. Oral magnesium sulphate may also be less effective as a laxative or supplement when given at the same time.
What this interaction means in plain words
Magnesium sulphate and Sodium polystyrene sulfonate have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Magnesium sulphate belongs to Cardiovascular; Sodium polystyrene sulfonate belongs to Miscellaneous.
Why it happens
The resin binds magnesium (and calcium) in the intestinal lumen, reducing magnesium absorption and increasing fecal loss.
What to do
Monitor serum magnesium, calcium and potassium during resin therapy and replace magnesium if low. Separate oral magnesium from the resin by at least 3 hours where practical.
