Summary

Fluoroquinolones can disturb glucose homeostasis and cause either hypoglycemia or hyperglycemia, particularly in diabetic patients. When given with a glucose-lowering agent such as vildagliptin, glycemic control may become less predictable. Moxifloxacin carries a lower dysglycemia risk than some other quinolones, and vildagliptin alone rarely causes hypoglycemia.

What this interaction means in plain words

Moxifloxacin and Vildagliptin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Moxifloxacin belongs to Anti Bacterial; Vildagliptin belongs to Endocrine.

Evidence level: Low

Why it happens

Fluoroquinolones affect pancreatic beta-cell insulin release, producing additive or unpredictable effects on glucose with antidiabetic drugs.

What to do

Counsel the patient to recognize symptoms of low and high blood glucose and check blood glucose more often during the antibiotic course. Switch to a non-quinolone antibiotic if dysglycemia occurs.
