Summary

Osimertinib is partly metabolized by CYP3A4, and strong inhibitors such as posaconazole can modestly raise its exposure. This may increase toxicities such as QT prolongation, interstitial lung disease and diarrhea.

What this interaction means in plain words

Osimertinib and Posaconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Osimertinib belongs to Oncology; Posaconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Posaconazole inhibits CYP3A4-mediated metabolism of osimertinib, increasing its plasma concentrations.

What to do

Prefer a non-azole antifungal when possible. If coadministered, monitor ECG and electrolytes and watch for pulmonary and gastrointestinal toxicity.
