Indications
Mécanisme d'action
Sphingosine 1-phosphate (S1P) receptor 1 modulator that binds with high affinity to S1P receptor 1
Posologie adulte habituelle (résumé de la notice)
Multiple Sclerosis (Initiation) Starter pack must be used for initiating treatment: 2 mg PO qDay initially; increase dose as per dose titration regimen (14-day titration)
Grossesse et allaitement
- Grossesse
- Unsafe EU label: contraindicated in pregnancy. In rats and rabbits it caused fetal death and malformations at clinical exposures without maternal toxicity (S1P receptor role in embryo development); another S1P modulator raised birth-defect risk in humans. Use contraception; stop 1 week before conceiving.
- Allaitement
- Unsafe No human milk data; rat offspring were exposed via milk. US label: weigh benefits; EU label: risk cannot be excluded, should not be used while breastfeeding. LactMed: long half-life and possible infant toxicity; experts advise avoiding the related fingolimod.
Effets indésirables signalés
- Somnolence
- Elevated hepatic transaminases
- Hypertension
- Urinary tract infection
- Dyspnea
- Dizziness
Contre-indications
- Presence of Mobitz type II second-degree, third-degree AV block, or sick sinus syndrome, unless patient has functioning pacemaker
- In the past 6 months, experienced myocardial infarction, unstable angina, stroke, TIA, decompensated heart failure (HF) requiring hospitalization, or Class III/IV HF
Précautions particulières
- Consider possibility of severe exacerbation of disease after discontinuing therapy; monitor for severe increase in disability upon discontinuation and appropriately treat; after stopping therapy in the setting of PML, monitor for development of immune reconstitution inflammatory syndrome (PML-IRIS)
- Elevated transaminases may occur; if symptoms (eg, unexplained nausea, vomiting, abdominal pain, fatigue, anorexia, rash with eosinophilia, jaundice, dark urine) occur during therapy, check hepatic enzymes; if significant liver injury confirmed, discontinue therapy
Informez votre pharmacien ou votre médecin en cas de grossesse ou d'allaitement, de maladie rénale ou hépatique, d'allergies, ainsi que de tout autre médicament ou complément que vous prenez.
