摘要
Long-term lansoprazole use can cause hypomagnesemia, and low magnesium (often with low potassium) increases susceptibility to QT prolongation and torsades de pointes with domperidone. The risk is mainly seen with prolonged PPI therapy and is greatest in older patients, those on diuretics or digoxin, and those with structural heart disease or other QT-prolonging drugs. The interaction is indirect and uncommon but potentially serious.
用白話說明這項交互作用的意義
Domperidone and Lansoprazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Domperidone belongs to Gastrointestinal Tract; Lansoprazole belongs to Gastrointestinal Tract.
為什麼會發生
Pharmacodynamic and indirect: PPI-induced hypomagnesemia (reduced intestinal magnesium absorption) lowers the repolarisation reserve, potentiating the QT-prolonging hERG-blocking effect of domperidone.
處置方式
Check magnesium and potassium before starting domperidone in patients on long-term lansoprazole, and correct deficits. Use the lowest effective domperidone dose for the shortest time (labelling advises a maximum of 30 mg/day and about one week for nausea/vomiting).
可向藥師詢問的問題
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
