Summary

Toremifene prolongs the QT interval in a dose-related way, and chloroquine has a similar effect. The combination increases the risk of torsades de pointes, especially in patients with hypokalemia, hypomagnesemia, bradycardia or congenital long QT.

What this interaction means in plain words

Chloroquine and Toremifene can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Chloroquine belongs to Anti Parasitic; Toremifene belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic QT prolongation from blockade of cardiac potassium channels.

What to do

Avoid the combination when possible, since toremifene labeling advises against coadministration with QT-prolonging drugs. If unavoidable, correct electrolytes and monitor ECG closely.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
