Summary

Higher gastric pH reduces absorption of posaconazole oral suspension, which depends on acidity, and may lower antifungal exposure. Delayed-release tablets are much less affected. Reduced exposure could compromise prophylaxis or treatment of invasive fungal infection.

What this interaction means in plain words

Esomeprazole and Posaconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Esomeprazole belongs to Gastrointestinal Tract; Posaconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Elevated gastric pH decreases dissolution and absorption of posaconazole suspension.

What to do

Prefer the delayed-release tablet or IV formulation when acid suppression is needed, and avoid the suspension with a PPI where possible. Consider posaconazole trough monitoring.

What to ask your pharmacist
