Summary
Toremifene can cause hypercalcemia, particularly in patients with bone metastases, and thiazides reduce urinary calcium excretion, which may worsen this. Toremifene also prolongs the QT interval, and thiazide-related hypokalemia or hypomagnesemia may add to that risk.
What this interaction means in plain words
Hydrochlorothiazide-irbesartan and Toremifene can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-irbesartan belongs to Cardiovascular; Toremifene belongs to Endocrine.
Why it happens
Thiazide reduction of renal calcium excretion adds to toremifene-related hypercalcemia; electrolyte depletion augments QT risk.
What to do
Monitor serum calcium, potassium and magnesium, and correct abnormalities. Consider a non-thiazide antihypertensive if hypercalcemia develops.
What to ask your pharmacist
