Summary
Rolapitant is a long-acting CYP2D6 inhibitor and can raise iloperidone concentrations for several weeks after a dose. Higher iloperidone exposure may add to QT prolongation, orthostatic hypotension and sedation.
What this interaction means in plain words
Iloperidone and Rolapitant can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Iloperidone belongs to Central Nervous System; Rolapitant belongs to Gastrointestinal Tract.
Why it happens
Rolapitant inhibits CYP2D6, reducing the metabolism of iloperidone.
What to do
Monitor for orthostasis, sedation and QTc prolongation, particularly in patients with other QT risk factors. Consider reducing the iloperidone dose if adverse effects emerge, and remember that the inhibition persists for at least 28 days.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
