Summary

Both drugs are azoles that prolong the QT interval, so combination markedly raises arrhythmia risk. Levoketoconazole is also a strong CYP3A4 inhibitor and its labeling warns against combining it with other QT-prolonging drugs.

What this interaction means in plain words

Levoketoconazole and Posaconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Levoketoconazole belongs to Endocrine; Posaconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Additive QT prolongation, with inhibition of CYP3A4 and P-gp by levoketoconazole possibly increasing posaconazole exposure.

What to do

Avoid concurrent use. Choose an alternative to one of the agents, and if unavoidable monitor ECG and electrolytes closely.

What to ask your pharmacist
