Summary

Tricyclic antidepressants and metoclopramide both can cause extrapyramidal effects, sedation and QT prolongation, and metoclopramide has been linked to serotonin toxicity with serotonergic antidepressants. Trimipramine's CYP2D6 involvement may also modestly affect metoclopramide exposure.

What this interaction means in plain words

Metoclopramide and Trimipramine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Metoclopramide belongs to Gastrointestinal Tract; Trimipramine belongs to Central Nervous System.

Evidence level: Low

Why it happens

Additive dopamine antagonism, CNS depression and serotonergic effects, with possible CYP2D6 competition.

What to do

Monitor for extrapyramidal symptoms, sedation, serotonin toxicity and ECG changes where risk factors exist. Use the lowest effective metoclopramide dose for the shortest time.
