Summary

Thiazide-type diuretics reduce urinary calcium excretion, and toremifene can cause hypercalcemia, particularly in patients with bone metastases. Combined use may raise serum calcium. Toremifene also prolongs QT, and metolazone-induced hypokalemia or hypomagnesemia can increase arrhythmia risk.

What this interaction means in plain words

Metolazone and Toremifene can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Metolazone belongs to Cardiovascular; Toremifene belongs to Endocrine.

Evidence level: Moderate

Why it happens

Thiazide-like reduction of renal calcium excretion adds to toremifene-associated hypercalcemia, and diuretic-induced electrolyte loss potentiates QT prolongation.

What to do

Monitor serum calcium, potassium and magnesium at baseline and periodically, and correct abnormalities. Consider an alternative diuretic, and obtain ECG monitoring if other QT-prolonging risks exist.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
