Summary

Combining an opioid with a serotonergic antidepressant can rarely cause serotonin syndrome, with agitation, tremor, hyperreflexia, clonus, fever and autonomic instability. Oxycodone has weak serotonergic activity, so the risk is lower than with agents such as tramadol or meperidine, but it is recognized in labeling.

What this interaction means in plain words

Oxycodone and Vilazodone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Oxycodone belongs to Musculoskeletal; Vilazodone belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive serotonergic effects from vilazodone (SSRI and 5-HT1A partial agonist) and oxycodone.

What to do

Use the lowest effective doses and monitor for signs of serotonin syndrome, especially at initiation or dose increases. Stop both agents and treat supportively if symptoms appear.

What to ask your pharmacist
