Summary
Stiripentol inhibits several CYP enzymes and can also induce others, so it may change exposure to co-administered drugs unpredictably. Ticlopidine is mainly metabolised by CYP2C19 and CYP3A4 and is itself a CYP2C19 and CYP2B6 inhibitor, so the net effect on either drug is uncertain. Clinically relevant changes in ticlopidine activity or in stiripentol exposure are possible.
What this interaction means in plain words
Stiripentol and Ticlopidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Stiripentol belongs to Central Nervous System; Ticlopidine belongs to Cardiovascular.
Why it happens
Stiripentol inhibits CYP2C19 and CYP3A4 and induces other CYP enzymes, while ticlopidine inhibits CYP2C19, so reciprocal metabolic effects are possible.
What to do
Monitor for platelet and hematologic effects of ticlopidine and for stiripentol adverse effects or loss of seizure control. Adjust doses according to clinical response and tolerability.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
