Summary
Megestrol may inhibit the renal cationic secretion of dofetilide, raising its plasma levels. Higher dofetilide exposure prolongs the QT interval and increases the risk of torsades de pointes. The dofetilide label advises against this combination.
What this interaction means in plain words
Dofetilide and Megestrol are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Dofetilide belongs to Cardiovascular; Megestrol belongs to Oncology.
Why it happens
Inhibition of renal tubular cation transport reduces dofetilide clearance, leading to additive QT prolongation.
What to do
Avoid the combination and choose an alternative to megestrol where possible. If it cannot be avoided, monitor QTc and renal function closely.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
