Summary
Enzalutamide is metabolized partly by CYP3A4 and CYP2C8, so posaconazole may increase concentrations of enzalutamide and its active metabolite, raising seizure and other adverse-effect risk. However enzalutamide is a strong CYP3A4 inducer and can reduce posaconazole exposure, potentially compromising antifungal efficacy.
What this interaction means in plain words
Enzalutamide and Posaconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Enzalutamide belongs to Endocrine; Posaconazole belongs to Anti Fungals.
Why it happens
Posaconazole inhibits CYP3A4 metabolism of enzalutamide, while enzalutamide induces CYP3A4 and UGT, lowering posaconazole levels.
What to do
Monitor for enzalutamide toxicity such as seizures and fatigue, and monitor posaconazole trough levels or antifungal response.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
