Metabolized to mesalamine by intestinal flora Mesalamine (5-aminosalicylic acid) has anti-inflammatory effect; active component of sulfasalazine, specific MOA unknown; probably inhibits prostaglandin & leukotrienes synthesis & release in colon.

Ulcerative Colitis: Colazal: 3 capsules (2.25 g total) PO TID (total dose 6.75 g/day x 8-12 weeks

- Safe Limited data with balsalazide; animal studies showed no harm. It releases mesalamine (5-ASA), which is widely used in pregnancy without increased birth defects. Continue when needed to control colitis.

- Safe No balsalazide milk data; mesalamine reaches milk in low amounts. Acceptable; watch the infant for diarrhoea, which has rarely been reported with 5-ASA drugs.
